![]() ![]() Makes appropriate referrals to other health care team members in a timely manner.Collaborates with members of the healthcare team when necessary.Documents and completes forms accurately, timely and according to TCHHN policy.Communicates in clear and concise verbal and written reports to the patient, family and health care team members the condition, course, and treatment plan, as well as, identifies psychosocial needs of the patient and family.Formulates a plan of care in collaboration with the family, physician, and other members of the health care team, which incorporates health care maintenance, discharge, and follow-up care.Prescribes medications and therapeutic devices as delineated by licensure and privileges.Provides interpretation of necessary laboratory and radiographic data for incorporation into the patient data base and progress evaluation.Develops and implements an initial plan of care for the management of presenting problem.Performs tests and procedures necessary to augment physical findings based on established protocols.Performs a complete physical examination and assessment of current health issues and discriminates between normal and abnormal findings.Elicits, records, and interprets a complete health history to include all present and past medical history, family, and psychosocial aspects of care.Demonstrates clinical assessment skills necessary to make pertinent and accurate assessments of the physical, psychosocial, spiritual, and educational needs of the patient.Provides care for the patient in collaboration with the collaborating physician, when necessary, and as directed by the delineation of privileges where applicable.If the APP will be practicing in the hospital setting, the delineation of privileges will also dictate the scope of practice. May provides general medical and/or surgical care and treatment to patients in a medical setting (office, telemedicine, hospital, Urgent Care, LTC, or Clinic) in collaboration with physician(s) for APRNs OR PA's where dictated. The Advanced Practice Provider (APP) may be an APRN OR PA that has been prepared with the appropriate graduate level education and authorized to practice and prescribe as such. ![]()
